To apply for a 75% EPF/EPS online withdrawal after one month of leaving the job, the date of entry/mark exit can only be entered on the EPFO site after two months. The question is whether the online form will still accept and process the withdrawal request without the date of exit (DOE). If it does, which form and subcategory should be used?

At the field asking for the amount, should I enter 75% of the PF plus the pension contribution of the employer as well, or is it only for the PF total part? Also, should I consider the 10% tax part in the calculation?
