Hello Everyone, can I get a feedback form for a new joiner regarding their work progress to be filled out by the concerned head of the department? This form is necessary for record-keeping and will help clarify whether we have hired the right candidate or not.

Design your form around the person description and position description for the job the person was hired for. You also need questions to be answered by supervisors or managers regarding whether the person is fitting in and working collaboratively with other staff members, etc.

This is not a difficult form to put together and should be within your skills to do it.

Finally, if you want to clarify whether you have recruited the right person or not, I'd suggest you have a problem with your recruitment process. It's too late once the person has started work for your organization. You need to get it right the first time.
