Req. Feedback form of a new Joiner about his progress

Can I get a Feedback form of a New Joiner about his work progress which is to be filled by concern Head of the department. This form is required to keep in record, which will clarify whether we have recruited a right Candidate or not

Design your form around the person description and position description for the job the person was hired for.

But you also need questions to be answered by supervisors/managers as to whether the person is fitting in, and working collaboratively with other staff members, etc.

This is not a difficult form to put together, and should be within your skills to do it.

Finally, if you are wanting to clarify if you have recruited the right person or not, I'd suggest you have a problem with your recruitment process. It's too late once the person has started work for your organisation. You need to get it right first time.
