PF transfer rejected due to member is non eps member

Hi,

I placed a request to transfer from my old pf account to new pf account.
But the transfer request got rejected with below reasons.
Please help me how to solve the issue.

Your Claim [ Claim Id - xxxxxxxxxxxxxxxxxx] has been rejected due to :
1) MEMBER IS NON EPS MEMBER, CONTRIBUTED PENSION, PLS SUBMIT REVISE FORM 3A BY THE EMPLOYER

When I checked with my new employer and old employer, they said the eps is active.
I see pension contribution is reflecting in passbook of my old and new pf accounts.

2) FATHERs NAME DIFFERS

Please help me how to solve the issues.

take clarification/undertaking letter of present employer and submit related pf office.
Also check with concern pf department for fast relief.
