Provident Fund withdrawal - contacted by an ex-employee running his own business for seven years

I have been contacted by an ex-employee running his own business for seven years. He wants to withdraw his Provident Fund.
I need help in finding out his UAN and the process to follow. He does have few statements but UAN did not exist
