What's difficult about filling in this form?
It is quite clear even to me what needs to be done, and I neither live or work in India, and have never seen such a form before.
Just fill in the details as described, e.g., names, dates etc.
You should know all this stuff if you are working in HR. Look back at the previous forms and see what the person before you did.
