PF claim rejected because the passbook does not contain an authorized signature

I have submitted a PF claim on the portal. The first time it got rejected because the passbook did not contain an authorized signature. I have corrected it with the seal of the bank and resubmitted it. Again, it got rejected, saying, "Claim Rejected - PASSBOOK NOT ATTESTED BY BANK." Please let me know the solution.

Please upload a blank cheque instead of the passbook containing your name in respect of the bank account seeded as KYC in your account.
