How To Change Employee Name In Epf Account - DOC Download

Dear Sir,
I' Kuldip kumar ojha S/o Lt. Birju Ojha my Pf Account No.JH/RAN/00/31594/000/0000167 U.A.N. No.100721885266, Contractor Name : Shri Gurugovind Singh Nursery/ (RO) Ranchi we are already send amendment of Name & Date of birth thru. contractor but it has not done correction of Name & date of birth till date.
Kindly amend the Name Kuldip Kumar Ojha & date of birth 05/07/1972 instead of Kuldeep Ojha & date of birth.

I' Devendra kumar S/o Asharam my Pf Account No.UK/DDN/00/36252000/11724U.A.N. No.100003171461, Contractor Name : jitendra kuma we are already send amendment of Name & Date of birth thru. contractor but it has not done correction of Name & date of birth till date.
Kindly amend the Name Devendra Kumar & date of birth 20/08/1991 instead of Devendra kumar& date of birth.
