Confused About PF Calculation in IT? Should Medical and Conveyance Be Included?

My salary structure is basic + HRA + medical + conveyance + special allowance. Then, for calculating PF, they are considering 12% of (Basic + medical + conveyance + special allowance). Is this correct? I have heard that medical and conveyance should not be included. Is that accurate? Also, why is DA not being calculated?

On DA, PF is payable. However, in your pay factors, DA is not included.
