Understanding CTC: Is Insurance Included and How to Calculate It with Only Professional Tax?

We are a firm of 15 people, hence no PF in CTC. My question is:

1) Is the insurance cost to the company part of CTC?

2) Can someone provide a calculation of CTC with only professional tax deduction?
