How Do You Choose the Best Employee of the Month or Year? Seeking Format and Tips

Can anybody provide me with the format for selecting the best employee of the month or year, along with any suggestions?

I am working in a hospital where we have nearly 2000 staff. We are planning to give the Employee of the Month award (other than doctors). However, I am finding it very difficult to assess, as we have nurses, clerks, pharmacists, housekeeping staff, etc., and the size is also very huge. Please help me in this matter. How can I proceed? Do you have any sample assessment format or any tests or interviews to suggest?
