Dear Neeraj-Dwivedi1,

ESIC certificate of entitlement in Form 105 has to be filled by the employer when any employee works in another location where his registered ESIC dispensary is not available. Simply, this form is used when an employee is on temporary duty or authorized leave. The employee has to take ESIC Form 105 from his employer, and it has to be submitted to the new ESIC dispensary. This Form 105 is to be downloaded from the IP portal.

Dear Akhil Bhartiya,

Thank you for your message. Below are the points related to the current situation:

Currently, the employee is in his hometown, which is in Madhya Pradesh (MP). He works in Maharashtra but requires medical treatment in MP.

Thank you for bringing this to my attention.

Best regards,
