Employee consent letter format - deducted 10% & 20% Salary of all our employees to pandemic situation Covid 19

Hi All, I am the HR - from a Hospitals, We have deducted 10% & 20% Salary of all our employees to pandemic situation Covid 19.
I want to take a consent from employees for salary deduction/reduction done by the organisation.
pls share any format if available.

Same require , my company also reduce salary during covid19 and ask to prepare consent form
