Performance appraisal form with Skills, parameters & questions - What is the appraisal percentage depending on 1 to 5 ratings?

Hi Connections, I am working in small IT company need to do appraisals, please guide me what should be the standard percentage increase in salary as per performance rating 1 to 5. If some one have performance appraisal form with Skills, parameters & questions, please share it.
