Is there any Specific format for appointment of ICC members?

I need to appoint members of ICC. Need help in draft of appointment letter.

Internal Committee Presiding Officer as well as Members are Nominated ( not Appointed) by the Employer/ Head of Establisment/Institution, from amongst Employees of at the Workplace and in addition One Non-Employee Outsider is Nominated as Member (External) for a Term of up to Three Years as prescribed under Section-4 of the SHWW (P,P &R) Act 2013.
