Struggling with Form 10D for My Dad's Pension: Missing Details and Need Expert Help

Hi all, in my father's case, he worked from 1993 for approximately 12 years. He doesn't know the date of relieving and the last salary withdrawn. After that company, he joined another company and resigned 6 years ago, and he received a PF final settlement. Now the issue is that I raised a grievance to the PF office in Madurai, stating whether my father's first company makes him eligible for a pension. I received a reply stating that yes, he is eligible, but he has to submit Form 10D along with proofs.

Issues:

1. My father's date of relieving is not known.
2. The last establishment owner is not available now.
3. Some details are unknown to fill Form 10D (like the relieving year, salary, and total pension).
4. "DETAILS OF BREAK IN SERVICE" form downloaded from your site. Can you please attach a sample filled form for user understanding?

Experts, please help with my issues.

You can try to get a scheme certificate from EPFO with some details which might have been available with your father, like EPF a/c number with your father's last company. Also, get a scheme certificate from EPFO for your father's second employment.

You can submit both to the PF office if your father attains 58 years of age along with relevant documents.

Now my father's age is 51. Is there any possibility to get a pension, or do we need to wait until he is 58 years old?

In Form 10-D, can we mention 'Type of pension' as REDUCED PENSION? Is this the correct way to apply?

You have the option to avail a pension at a reduced amount if your father did not attain 58 years of age. However, you will get the full pension if he submits the necessary documents to EPFO after he attains 58 years of age.

In the case of reduced pension, you will still have to wait until your father attains 52 years of age.
