Procedure of withdrawal of PF when it is deposited in trust?

I have a query regarding the procedure of withdrawal of PF when it is deposited in a trust. It seems very complicated.

Please provide full details about your status, including how long you have been working at this firm, your membership in the EPF, and information about your employer - specifically, whether the PF is administered by a Trust established by the employer in accordance with the bye-laws approved by the EPFO, or if contributions are remitted to the EPFO.
