Provide me a format for contract employee payslip and freelancer payslip format

Please can anyone provide me a format for contract employee payslip and freelancer payslip format. Threshold limit for deductions of ESI and PF. Thanks in advance.

Contract employee should be paid as per minimum wages what ever is prescribed in your state enactments, Pay slip can be designed by yourself considering the structures provided.
Basic,DA or VDA are to be included without fail if following MW structure.
ESI & EPF thresholds are provided in the statutes with latest amendments time to time.
