Can someone share the Provident Report Format & ESIC Report Format according to Govt. Norms

Hi, Can someone share the Provident Report Format & ESIC Report Format according to Govt. Norms.
Formulas of Columns Mentioned in PF Report, if shared would be very helpful like EPS, EDLI & Admin Charges Formula.

Now both ESI & PF are online. All information available in the portal itself.

ESI:
You need to enter the attendance days and total salary earned online. ESI contribution (both employee as well as employee contribution) will be generated automatically.
PF:
You need to download ECR file from EPFO website. You can learn the procedure from youtube.
PF Admin charges (A/c no. 2) - 0.5% of PF basic salary or Rs. 500/-, whichever is higher
EDLI charges (A/c no. 21) - 0.5% of PF basic salary
EDLI admin charges (A/c no. 22) - 0.01% of PF basic salary or Rs. 200/-, whichever is higher

Attached the PF worksheet which was missed in my earlier response.

Earlier, EPFO Ac no 22 has been reduced from 0.01% to 0%.
