Form 11 was introduced in India under the Employees' Provident Funds and Miscellaneous Provisions Act, 1952. It came into effect on April 1, 2012. Prior to the implementation of Form 11, employees were required to fill out Form 13 for transferring their PF account from one employer to another. Form 11 serves as a declaration form for the new employee to provide basic details to the employer for PF registration purposes. It includes information such as name, date of birth, contact details, previous employment details, and nomination details for PF benefits.
