Attachment Letter - Doc Download

This is Tejesh, working as HR in Pharmacy company.
I started employee satisfaction survey in our organization and i made a formatt of the survey form.
so now i want to send it to all staff allover India, so i have to attach a attachment letter along with the survey form.
so i request u to send the attachment letter formatt, so that i can send it to our staff.
Iam the only HR in our Organization, Iam sending you the formatt of simple statisfaction suvery formatt...

I m sorry I dont hav any attachment letter right now but I would like to know what will be the next step after this.That means after the survey is over.
