Thank you for your kind response. Following are the forms that I am looking for:

1) Work report Excel Sheet for our recruiters/bd
2) Client requirements form which will be maintained by BDE and key accounts HR in a consultancy.
3) Total MIS report format that should be maintained by HR Recruiter.
