Please advise if an employee wants to change the local address for ESIC benefit so is there any form or declaration is needed by the employee? if yes kindly assist.

Dear Mr Upendra Sharma, Employer has to log in to ESI Portal to edit Details of Insured person.Insured person can not change Address
