Which documents are required to be submitted for POSH annual return and where these documents are required to be submitted?

Can anybody tell us which documents are required to be submitted for POSH annual return and where these documents are required to be submitted?

For Preparing the Internal Committe's Annual Report for Calendar Year 2018 under Section-21 of SHWW (P, P &R) Act 2013, please assist the IC to
i) Collect the Data & Details from all HODs, collate them & present under the prescribed Five (5) Headings indicated in Rule-14 of the Central Rules framed under the said Act and gaztted on 9.12.2013. Keep all related Reports /Emails etc in the custody of the Internal Committee Presiding Officer.
The Finalized Annual Report one Copy to be submitted / Forwarded to the Employer for further action under Section-22 and another Copy Filed before the District Officer (Section-5). No Documents as attachments need to be enclosed forwarded
