My name is Chaithanya from Hyderabad. Currently, I am working with a telecom-related company. Here, I have to implement a performance appraisal for 700 employees from different departments (HR, Finance, Sales, Customer Care, Back End, Collections, Technical, Networking). I would appreciate your valuable suggestions on how to conduct the performance appraisal for these 700 associate employees. If you have any format or form available, please send it to me. First of all, you need to create different departments, and based on that, you can create the performance forms. You can include the following information in the respective forms:

Employee Name:
Designation
Department:
Date of Joining:
Due Confirmation Date:
Review Date (when it was actually done)
Name of the Appraiser:
Designation of the Appraiser

At the bottom of all the pages:
Signature of the Reporting Authority:
Signature of the Appraiser:
Signature of the Appraisee:
Signature of the HR:
Date:
