Withdrawal EPFO - UAN password misplaced & also the mobile number which is linked with my UAN misplaced

Sir/Madam, My UAN password misplaced & also the mobile number which is linked with my UAN misplaced. How can i apply pf withdrawal.

click on forgot password and when it displays the mobile number, opt to change the mobile , put new mobile and fill requisite details plus they will ask for one KYC seeded document details and will send otp to new mobile, put the otp and set password, new mobile is linked as your a/c is ready for all transactions
