Creating Unique Self-Appraisal Forms for Different Levels in a BPO: How to Avoid Repetition?

Dear All,

I am employed in the HR department of a BPO. We have six levels of Associates (call center operations) and other general staff/departments. I need to create Self-Appraisal Forms for all six levels. How can I create so many formats without repeating questions?

In Form I, I have already included the following questions:
- Performance-related questions
- Suggestions, etc.
