IP no. 7700006420. Can anyone tell me why this IP is not entitled for medical benefits?

Oct 17 and jan to march 18 no contribution is paid , total paid days was only 61 while the eligibility stands for 92 days minimum

My brother had met with an accident while at work and he is covered under wait scheme..after the accident he was taken to ESIC empanelled hospital..doctor has told him that he needs to undergo surgery to his leg which is due in 2 weeks time. What is the procedure to avail the medical benefit under esi
scheme?

But dear U S sharks sir that IP has worked for all the six monthsand the received salary for all the six months each month from October 2017 to March 2018.. it is the fault from the employer that they did not submit the monthly contribution regularly. What to do about it now?
