Struggling with PF Transfer? Need Help with the Latest Covering Letter Format!

I am facing a peculiar issue. I am planning to transfer my PF from the previous employer to my current employer. My previous employer is asking for a set of documents which comprises an attested Form 13 and a covering letter. I have generated Form 13 from the UAN portal. The covering letter format is not available to me. I work for a startup, and my current employer also does not have it. My previous employer mentions that the basic format of the covering letter has been changed. It seems that the payment instruction needs to be added to the letter. I am requesting everyone, if anyone has the latest format of the covering letter, please share it with me. Thanks in advance.

No letter is required to submit Form 13 provided the KYC is approved digitally by the previous employer. Simply make a photocopy of Form 13 after the employee and the previous employer have signed it, and get the photocopy stamped by the department. Fill out Parts A, B, and C carefully, ensuring not to leave any column blank.
