Training Feedback Survey - do not want to go with the generic questionnaire that covers the basic feedback

We have recently switched on to online training from class room training sessions for our employees and now we want to conduct a feedback survey. Would want your contribution on what kind of questions can we add which will result in a constructive and rational feedback. We do not want to go with the generic questionnaire that covers the basic feedback.
One such set would be like below but we are trying to develop questions beyond these. Please guide and suggest.
Example:
Q1. Participation and interaction was encouraged
Q2. The topics covered were relevant to me
Q3. The Audio & Video quality was good
Q4. This training experience will be useful in my work
Q5. I will participate in other sessions in future
Q6. I will recommend this training course to a colleague?
Q7. Do you have any other comments, questions or suggestions?

Your questions are good and generic, however you need to remember that you re asking for feedback for an on line course and your objective is to improve the course/training's overall effectiveness.
1. Relevancy of Course content
2. Structure (flow)
3. Presentation (audio-video)
4. Easy-difficult to understand
5. Duration adequate enough to engage
6. The voice is clear and language easy to understand
7.Could this topic better covered by other training methodology if yes please suggest
8. Does this course offer valued take away
