Online PF Shows "Settled" but No Money Yet – How to Correct Account Details and Submit ASR Form?

I applied online for PF and it is showing “settled” in status but no money credit yet as there were some misktake in account details so know I have updated the detials. I got mail after Follow up from PF saying that needs to fill n Sumit physically ASR form to PF office,
So now how to get n fill that form and what other documents are required to do so.
