How Do I Correct an IP Name Error in ESIC? Need Help with the Letter Format

One of IP's names is wrongly inserted in ESIC. Now we have to correct this name.

What should be the format of the letter to give for ESIC Office (Is it the same as the Joint declaration for PF name correction or different?)
