What should be the format of letter to give for ESIC Office to do correction in IP Name?

One of IP's name is wrongly inserted in ESIC. Now we have to Correct this name.
What should be the format of letter to give for ESIC Office ( I=as same as Joint declaration for PF name correction or different?)
