PF portal - Need to change Exit date - mistake in employee's exit date

We have mistakenly mentioned exit employees exit date in PF portal, that created an issue for them.
So we need to change the exit date for them, please guide me, I need your help.

For all those employees, you need to submit the joint declaration form to the epfo duly signed and attested by both the employee and the employer.

In addition to the reply by Mr. Rahul, Please also submit form 5 with the same. Look for the new format available with PF office.
