Format for meetings schedule form maintenance by HR for all departments?

We are going to implement the weekly meetings schedule form maintaining by HR. Already meetings all running regularly but there is no proof , reports and filing system. so weekly plan meeting with reports status need to maintain by HR. for that i need template and idea. we have 3 departments Production, Sales, Operations. i will attach the topic for example. Day, Department, time, meeting, organiser, participants. For reports separately like this i need proper words for this. i have attached the template. please do the needful.

Suggest add further details like place, which all appointments will attend, date by which agenda points are to be submitted.
Keep fixed dates and times for meeting and circulate schedule on monthly basis to all concerned with copy to your HOD.
Minutes of meeting must be prepared and sent to all concerned for action and feed back.
