How Do I Prepare These Essential Documents for Ending a Manpower Service?

hi everyone i'm from Gurgaon Haryana n working in a logistics company, we are also working in manpower, now we are leaving manpower service of one customer, at that they required below document
• Declaration of PF remittance
• Declaration of ESIC remittance
• Declaration of PF & ESIC (on letterhead)
• Muster Roll and Wage register
please help me to provide format of above document and how to make them

Yes, I have a PF and ESI number, but I just don't know how to give a declaration of PF and ESI remittance. I also don't have any idea how to prepare a Master roll or wage register. I need to share these documents with my customer tomorrow morning. If you can help me with this, I would be thankful to you.
