How to write an email to my boss to ask for a salary increment?

I need to write an email for raise in my salary, my boss told me to write an email.
initially i was told that my salary will be revised after completing probation period of 3 month but now its almost 2 years and i dint get any increment how to write an email pls help

What is the system in your company for increment ? can you check why your boss asking for increment email, is this way for increment in your company. Please approach HR as well.

This is very non- traditional , outlandish requirement of your boss bordering on idiosyncrasy.
He is not accepting your verbal submissions and wants you to write an email justifying why you should be given increment.
So write to him stating your achievements during this period supported by facts and figures. Be confident and do not hesitate to ask for raise commensurate with your contribution.
