How to write email request to IT department for new system and email id creation for new joinee?

Hi Can any one help me on how to write email request to IT department for new system and email id creation for new joinee?

Hi,
If you are working in MNC, you need to raise request through ticket to IT Request Team via ticketing tool server.
If you are working in non-mnc org, I believe you need to mark cc to your admin also while raising request.
You need to provide IT with following details:
DOJ of new joiner
Emp Code
DOB of new joiner
Location and State
Grade of new joiner
Replacement or Fresh type of source (if replacement of resigned employee, share previous employee's system id details)
