Is This Medical Clause in Appointment Letters Fair and Clear? Seeking Urgent Advice

Hello, please suggest to me: can I include this point with the same words in the appointment letter? Is it correct language?

"In case of serious illness in which you have undergone treatment for more than 21 days, you will have to submit the complete reports to the company before joining your duties. If you are undergoing continuous treatment, you must not hide any facts and inform the company before joining your duties. In case of hiding the fact, the company can terminate your services without any notice with immediate effect for concealing the information."

Please advise me as this is very urgent.
