Issue related to Pension account

My pf was deducted by Ranbaxy Mohali and submitted to trust account at Delhi . I joined the company in 1991 and I have left the company in 2008 and my PF was settled through the company and it had been transferred to my bank account but i do not know about my pension fund as i know the pension was submitted in epfo office Chandigarh.Ranbaxy is unable to detect the issue currently.
My query is that sir how can i transfer this Pension account to my existing company and even i do not know the pension account number. Pls help me.

Dear Mr Kumar Sanjay, Please contact trustees of your previous Employer & submit PF Tranfer Form # 13 (Revised) to be submitted to E P F O- Chandigarh for Transferring your EPS Account to your Present EPS Account.you should have initiated action in 2008 itself. Nevertheless, submit your Transfer Form
