Email drafting - Notice period

I am Lakshmi. i am working as a HR executive in Pharmaceutical Company. Recently I joined in the organization
Now one employee is ready to leave without working notice period.my boss wants me to draft a letter saying that he wants to serve the notice period . any one please help me in this.

You can write as follows
Address the mail to employee by name,designation and employee number
Sir,
Reference is made your email/letter dated asking for voluntary exit from the company.
In this connection, your attention is drawn to para--- of the appointment order dated--- which specifically requires you to serve a notice period of ---- days.
Kindly adhere to laid down instructions while submitting your application for exit through proper channels
