Where I should apply for ESI registration, In Rajasthan or Haryana?

I have some queries for ESI coverage. My company is into Agriculture industry and I am hiring 10th employee next week and hence would be eligible for ESI coverage. We have office in Gurugram and plant in Bhiwadi (Alwar, Rajasthan).
1. Where I should apply for ESI registration, In Rajasthan or Haryana?
2. Is it Online or Offline mode of application?
3. How much time it takes for registration?
4. What all documents are required for registration?

1. Where I should apply for ESI registration, In Rajasthan or Haryana? You can take one main code from Bhiwadi and one sub-code for Gurugram (If registered office is inGurugram, take main code in Gurugram).
2. Is it Online or Offline mode of application? It is online process
3. How much time it takes for registration? 3-6 days
4. What all documents are required for registration?Registration certificate, Address proff, Employees details, Aadhar card, Pan Card etc.

I have been trying for shram suvidha portal but it is asking for 20 employees' list for common registration for ESI and PF but no option to go ahead for ESI.
For registration, website is not responding.... this has been observed for last 10 days.
