Hw to track & record IN time & OUT time of employees & also maintain Leave record?

Can anyone tell me how to track & record IN time & OUT time of employees & also maintain Leave record along with that.
In short i am looking for an excel sheet which can help me in maintaining IN & OUT time which has formulas to calculate.

What is your employee strength? Do you have any Bio-metric devices you use to track login/logout/break timings of your employees?
If you do it the old school way, attached is a copy of attendance tracker that serves the purpose.
