PF Employer contribution if Basic Salary is more than Rs.15000/p.m

Suppose an employee is getting Rs.20000 per month as Basic Salary,then what will be the contribution of Employer and employee in PF A/c No.1,A/c No.2,A/c No.10,A/c No.21,and A/c No.22.
Pls explain the both the scenario as DOJ-01.04.2014 and DOJ-01.04.2015.
