Require the EPF Bifurcation detail of employer site and employee site

Hi,
i would like to inform you that i have to require epf bifurcation details hereby request you to please provide to us.

12 % of Basic+ DA from employee side
&
13.15% of Basic + DA from employer side
From this 13.15 %
8.33 % of Basic considered for pension goes to pension Fund . if Basic+ DA>15000 then 8.33 of 15000/- goes to pension Fund i.e. 1250/- else if if Basic+ DA>6500/- then 8.33 of 6500/- goes to pension Fund i.e. 541/-.
0.65 is Administration charges
0.50 is for EDLI
Rest amount is for PF employer contribution.
