Please find the attachment copy under ESIC. Shall the eligible insured woman follow the attached procedure to avail maternity benefits? Is this procedure sufficient for availing maternity benefits, or are there any additional steps I need to include in the attached procedure?

Hello, my question is that I am staying in Ghaziabad and want my delivery to happen in Uttarakhand. The issue is how to apply for ESIC for maternity salary policy. Also, I want to go to a private hospital. If I choose a private hospital, will I be eligible for ESIC benefits? My concern is for the private hospital plus salary for 6 months eligibility. Please explain the whole procedure to me with full details.
