The idea is good, but initially, you need to have a list of activities you do for the employee like work culture, employee welfare, employee motivation, employee engagement activities, including performance management system, etc. Then you can link all those activities with the survey questionnaire. You can draft yourself on the same basing on the activities and circulate individually either through email or hard copies and get feedback.
