Our company is running with 2 branches in Chennai and having 2 separate registration addresses. Now, we have combined both into one and shifted to another place. What is the procedure to change the address in ESIC and EPF?
You need to write a letter regarding the same to both departments along with the proof of the changed address, the signature of the authorized signatory as per PF & ESIC departments. Apart from that, you need to submit Form Revised 5A (online and offline as well). Please check with both departments as well.
