Contract labour agreement

Hi,
I wanted to know whether contracts between the principal employer and the contractor for employing contract staff contain the salary of the contract employees as a part of the agreement or it to be separately agreed through written correspondence. Because including the salary of the employees will give an impression that they are the employees of the principal employer.

Does anyone have a sample agreement for appointment of contract employees.
