How to share the letter to collect the PF amount which is not settled for years?

Please help me to send a sample letter to all the left employees to collect the PF amount by submitting the PF Claim Forms.
Note : The employees are left on 2004 , 2013, 2014 and 2015 and not collected the PF amount.
The reason to send the mail because the trust fund will go to RPFC Welfare Fund.
