Seeking Transfer on Medical Grounds - Requesting Email Format to HR

Hi All,
I am fresher. I have been tagged to a long term project for 2 months now. I need a transfer from my current location to another location in same organisation on medical grounds. Please HELP ME with an email format to HR.

There is no prescribed format for the purpose. The application for transfer depends on your illness and the company policy regarding the same. Unless these essential matters are mentioned no meaningful draft could be prepared.
