Understanding Form U and Maternity Benefit Returns: Are They Only for Contract Labour Act?

Can anyone explain if Form U and maternity benefit annual return are required to be submitted only when the Contract Labour Act prevails? I am from Bangalore. Kindly advise.

Hi,

If you have Shop & Establishment registration, then you need to file Form U annual return every year. Maternity benefit return is included in Form U (Combined annual return).

Dear Seniors,

I have to fill Form U, and I am stuck on Serial Number 6A. Please help me with the following three queries:

1. "No. of Days worked": How do I calculate the number of days worked out of 365 days?
2. "No. of man days worked during the year": How do I calculate the number of Man Days Worked?
3. "Total Amount of salary/wages paid including O.T. wages and Allowances": Should I mention Gross Salary or Take-Home Salary in this cell?

Please assist me in resolving these issues.
